This includes coverage for two adults and one child, under the age of 18.
Young Adult Membership:

Why charge a membership fee?
The monthly membership fee allows Peak Performance Medical to provide the primary care services for a simple, fixed price. Much like Netflix or a gym membership, it allows our members to better budget their healthcare finances; without worrying about extra costs, co-pays and deductibles!
Is Direct Primary Care Insurance?
No, a Peak Performance Medical Direct Primary Care Membership is not health insurance. Instead, it is more of an “assurance” program. For a low monthly fee, you and your entire family, have access to quality healthcare when you need it and on YOUR schedule.
Why would I pay ‘extra’ fees for health care?
We do not think of our services as “extra”; rather an independent, alternative and better way to receive your primary care. Most of our members save money when combining our Direct Primary Care Membership with a high-deductible health insurance plan or with one of your partner’s medical cost sharing plans.
Is Direct Primary Care Membership available to patients with existing medical problems?
Yes. Our memberships (eligibility or fees) are not based upon any existing medical issues or conditions.
Can you manage chronic pain?
There are many modalities available to help minimize pain of chronic conditions and aggressively work with people to achieve adequate comfort. Our office takes a cautious and conservative approach to using narcotic/opiate medications such as hydrocodone, morphine, Lortab etc. and will only use them long-term in select situations and after consulting a pain management provider.
What about mental health problems?
Peak Performance Medical can offer primary care services for most any mental health problem, including associated medications. However, our office generally has a conservative and careful approach when using medications (particularly benzodiazepines such as Xanax or Valium) in this area and strongly promotes the use of non-medicine therapies and lifestyle changes.
Do I have to be a member? Can I just have a visit without becoming a member?
To receive all of the benefits of Direct Primary Care, you must be a member. However, non-members still have access to our OnCallMD App.
Are there any contracts or commitments? What if I decide to cancel?
We do require all patients to sign a contract. However, the contract can be canceled at any time after the initial thirty days, provided that the member contacts our office at least ten days prior to their next scheduled payment being due.
May I re-start my membership after canceling?
Yes, we do allow for re-enrollment after cancellation, but members will be charged a re-enrollment fee of $200 per member before service can be restarted.
Are there any hidden fees?
No. We provide most care to members without any fees at all! If we do charge for non-covered services, we always list prices upfront. If we recommend outside services, we try very hard to find you an upfront, reasonable price if paying cash. All you pay is your monthly membership fee and your initial enrollment fee of $40.00.
What about lab work and prescription drugs?
Peak Performance Medical Direct Primary Care Members receive in-house diagnostic testing for free. This includes rapid strep and urinalysis. For other lab services, we have partnered with several area labs that offer our member’s affordable cash based pricing. Through our exclusive partnership with an area pharmacy, we are able to offer our members wholesale pricing for most prescription drugs. This wholesale pricing results in many of our members paying less than $5.00 per prescription.
What if I need outside care? Specialists, diagnostic testing, ER, hospital?
We can order or refer you just as any insurance-based doctor can do. For some outside services, we can help find reasonable "cash" prices if you are paying out-of-pocket. We partner with Community First Urgent Care to offer Urgent Care Services when needed. This service is free to our members. That said, we do recommend having catastrophic health coverage in the case of an unexpected, expensive event such as surgery or hospitalization.
What other discounts do members receive?
Great question! Peak Performance Direct Primary Care Members receive a 30% discount on all Peak Performance Plus Services. In addition, members receive between a 10% - 20% discount at other area businesses that promote health and wellness.
What are your office hours and locations?
Simple, when our members need us we are available. Just contact our office or your provider (using the OnCallMD App.) and let us know you would like to be seen or that you have a question. This is healthcare on YOUR schedule. Currently we have two office locations in the Cincinnati area, with additional satellite locations coming soon. We also have our corporate headquarters in Louisville, Kentucky.
